06-011 Injured Worker Report_2014I PROCESS: TILLSONBURG FLEET SERVICES-INJURED WORKER REPORT i POLICY: F002 ! All injuries, occupatlonal illnesses, property damage and incidents (near misses) must be reported to the supervisor by the employee I SCOPE: All Fleet Department Employees I PROCEDURE: I 1) 2) A) BJ Immediately after an accident/incident, the supervisor shall ensure the safety of employees, public, equipment and facilities from further injury or damage and follow the steps laid out in this procedure. There are four categories of Injuries: (a) No Treatment (b) First Aid (c) Medical Aid ( d) Critical No Treatment Injury: 1) A 'No Treatment Injury' occurs when there Is an injury that does not require any treatment (i.e. bruised finger) 2) The employee will report the Injury to the supervisor 3) The supervisor will record the injury in the "FIRST AID REPORT BOOK", which Is to be kept at the First Aid Station. First Aid Injury: 1) A 'First Aid Injury' Is an injury that can be treated at the work site and does not require treatment from a health care professional (I.e. a cut finger that requires a band-aid only) 2) First aid treatment will be provided and the treatment recorded in the "FIRST AID REPORT BOOK" 3) The employee is to report the injury to the supervisor C) Medical Aid Injury: 1) A 'Medical Aid Injury' is an Injury that requires treatment (i.e. a cut finger that requires stitches from a health care professional (i.e. physician, chiropractor etc) but is not of a critical nature 2) The supervisor is to arrange for: a) first aid treatment for the injured employee and record the treatment in the "FIRST AID REPORT BOOK" b) transportation (i.e. private vehicle ,taxi, ambulance etc) of the employee to a location where professional health care can be delivered (I.e. doctors office, hospital) The Corporation of the Town of Tlllsonburg Management of The Corporation of the Town of Tillsonburg is vitally interested in its employees' health and safety. Our objective is to protect ourworkers from injury and illness. The Corporation of the Town of Tillsonburg, as the employer, is ultimately responsible for worlrer health and safety. I, David Calder, CAO of The Corporation of the Town of Tillson burg, am personally committed to taking every reasonable precaution for the protection of all workers. To fulfill this commitment The Town of Tillsonburg will make every effort to provide and maintain a safe and healthy workplace by adhering to acceptable industry standards and complying with occupational health and safety legislation. In ki:eping with occupational health and safety legislation, a healthy and safe worlcplace will be accomplished in consultation and cooperation with management and employees, in particular the worker health and safety representative. Supervisors will be held accountable for the health and safety of workers under their supervision. Responsibility includes ensuring that machinery and equipment are safe and that work practices are in compliance with established legislation, workplace practices and procedures. To protect their health and safety, workers must receive adequate specific work C) task training. Every employee ll)Ust protect his/her bealth and safety and the health and safety of other workers by working in compliance with legislation and established workplace practices and procedures. Contractors will be required to operate according to legislative requirements using best practices and following The Town ofTillsonburg policies and procedures regarding health and safety. Health and safety is an integral part of the Town's every day business. It is in the best interest of all to join together and put into practice health and safety principles in all work activities. This policy will be reviewed on an annual basis and revised as necessary.
